The Chronic Disease Management (CDM) Programme in General Practice was established in Ireland in 2020 and is showing significant positive impacts on patients risk factors for chronic disease and chronic disease control. A strand of the programme offers screening to a sub-section of the population at high risk of developing CVD and/or type 2 diabetes mellitus.

Familial hypercholesterolaemia (FH) remains critically underdiagnosed across Europe despite being a major and preventable driver of premature cardiovascular disease. FH affects approximately 1 in 300 individuals and is associated with lifelong exposure to elevated LDL-cholesterol from birth, substantially increasing the risk of early atherosclerotic cardiovascular disease.

Good afternoon, First of all, I would like to say that it is a privilege to participate in this EU Call for Evidence. As a citizen, I am deeply grateful for this opportunity. On behalf of my colleagues at our research company, I am pleased to submit a brief evidence-based report on the importance of population-based and systematic screening for valvular heart disease.

Cardiovascular and oral diseases share common risk factors, including poor diet, tobacco use, and harmful alcohol consumption. Tackling these shared risk factors through population-level policies such as food reformulation, sugar reduction measures, and taxation of tobacco and alcohol is essential to improving both cardiovascular and oral health.

Waiting lists currently represent one of the most critical organizational challenges for the Italian National Health System (SSN), with substantial consequences for patient outcomes, healthcare sustainability, and equity of access.

Position Statement on the European Commissions Initiative Council Recommendation on Health Checks for Cardiovascular Diseases We, as the patient organisation Initiative Herzklappe, strongly welcome the European Commissions initiative on health checks for cardiovascular diseases under the EU Safe Hearts Plan.

On behalf of the Society of Thrombosis and Hemostasis Research (GTH), I would like to express our appreciation for and strong interest in the Council Recommendation on health checks for cardiovascular diseases, which promotes an EU-wide approach to early detection and screening.

I welcome the European Commission's initiative to promote coordinated cardiovascular health checks across Member States. However, we note with concern that the current proposal does not explicitly address thrombosis, haemostasis, or venous thromboembolism (VTE) conditions that are intrinsically linked to cardiovascular disease and represent a major, often preventable, burden of morbidity and mortality across Europe.

Heated Community Hub welcomes the European Commissions initiative for a Council Recommendation on cardiovascular health checks and supports the objective of strengthening early detection and screening across the European Union. Cardiovascular diseases remain one of the most serious public health challenges in Europe.
